To be seen promptly, please arrive 15 minutes prior to your appointment to finish any paperwork and complete insurance information. Filling out the below forms ahead of time will speed up the process.
Bring both glasses and contact lenses to the appointment. Have your glasses on prior to seeing a technician.
Bring an updated medicine list of all your medications.
